Multi-center recruitment of about 240 people, including dissociative identity disorder, PTSD, trauma-exposed without disorder, and healthy simulation controls, about 60 each; 12-month follow-up with daily state records and a clinically approved mild threat/safety cue task, measuring identity states, beliefs, heart rate variability, skin conductance, and cortisol. Pre-registered tests examine whether threat precedes switching and whether switching precedes reduction in physiological load; bodily protection must be evaluated with independent injury or health endpoints, not subjective numbness.

Specificity of Seth's formulation
Threat-triggered state switching overlaps with existing dissociation theories; 'to protect body structure' is an additional functional assertion requiring long-term physical outcomes, not just brain-imaging differences in identity states.
Note
This 2016 paper contains an extended analysis of an existing PET sample and cannot be counted as an independent replication along with the earlier report; everyday contradictory beliefs also cannot be equated with dissociative identity disorder.
